These Terms of Service ("Terms") govern your use of adhdtelemed.com (the "Site") and the telehealth services provided by ADHD Telemed, LLC ("ADHD Telemed," "we," "us," or "our"). By using the Site or booking a visit, you agree to these Terms.
1. Our services
ADHD Telemed provides telehealth evaluation and ongoing care for attention, focus, and related symptoms that may be consistent with ADHD or other conditions, for patients age 13 and older. Care is delivered by licensed physicians by secure video and other telehealth tools.
2. Eligibility and location
You must be 18 or older, or age 13 to 17 with the participation and consent of a parent or legal guardian. You must be physically located, at the time of each visit, in a state where your treating physician is licensed (currently Florida, Georgia, Colorado, Virginia, Texas, Minnesota, Michigan, Ohio, and California). You must truthfully identify yourself and your location, and we may verify both.
3. Not for emergencies
ADHD Telemed is not an emergency service. If you have a medical or mental-health emergency, call 911 or go to the nearest emergency room. If you are in suicidal or emotional distress, call or text 988. Our services are not appropriate for active suicidal or homicidal thoughts, severe intoxication, acute psychosis or mania, chest pain, fainting, or other urgent conditions.
4. No guarantee of diagnosis or prescription
Your fees pay for professional clinical time, evaluation, medical decision-making, and documentation. Payment does not guarantee a diagnosis of ADHD, a prescription, a controlled substance or stimulant, a dose increase, a refill, a letter, or an accommodation. Whether to diagnose or prescribe is solely your physician's clinical decision. Your physician may decline to prescribe, request prior records or collateral information, check your state prescription monitoring program (PDMP), require a controlled-substance agreement, recommend in-person evaluation or testing, or refer you elsewhere.
5. Controlled substances
Controlled medications, including stimulants, are prescribed only when clinically appropriate and in compliance with federal DEA rules and state law, including any in-person visit requirements. Continued prescribing requires scheduled follow-up and monitoring. Missed follow-up, inconsistent information, or signs of misuse or diversion may result in medication being paused or discontinued.
6. Fees, membership, and billing
- Adult initial evaluation: $159 — a $49 deposit is charged when you book, and the $110 balance is charged after your visit is completed. After your evaluation, the adult ongoing-care membership is $150 per month, cancel any time.
- Teen and child evaluations: $545 for the initial evaluation, then $189 per month.
- Additional visits: medication management visit $150; 30-minute follow-up visit $100.
Prices are confirmed at booking. Pharmacy, laboratory, outside records, and in-person care costs are separate.
7. Cancellations and refunds
Cancel or reschedule at least 24 hours before your visit to keep your deposit. Missed visits or late cancellations may result in forfeiture of the $49 deposit. You may cancel your membership at any time; cancellation stops future monthly charges. Once a visit has started, charges are final, and refunds are not issued because of disagreement with a diagnosis, treatment plan, or decision not to prescribe.
8. Insurance
ADHD Telemed is a direct-pay practice and does not bill insurance, Medicare, or Medicaid. On request we provide a superbill you may submit to your insurer; reimbursement is not guaranteed. HSA and FSA cards are accepted.
9. Screeners and website content
Our online screener and articles are for education only. They are not a diagnosis and are not a substitute for an evaluation by a licensed clinician.
10. Your responsibilities
You agree to provide accurate and complete information, attend visits from a private location, keep your login credentials secure, take medications only as prescribed, and never share or sell medication. You may not use the Site to obtain medication under false pretenses, submit another person's information, or interfere with the Site's operation.
11. Ending services
We may decline or discontinue services for clinical safety reasons, legal restrictions, nonpayment, false information, suspected misuse or diversion, or abusive conduct, and will provide appropriate referral or emergency instructions when clinically indicated.
12. Disclaimers and limitation of liability
The Site is provided "as is." To the extent permitted by law, ADHD Telemed is not liable for indirect, incidental, or consequential damages arising from use of the Site. Nothing in these Terms limits liability that cannot be limited under applicable law.
13. Governing law
These Terms are governed by the laws of the State of Mississippi, to the extent permitted by applicable law. Clinical care is governed by the laws and professional standards of the state where you are physically located at the time of your visit.
14. Changes and contact
We may update these Terms and will post changes here with a new "last updated" date. Questions: hello@adhdtelemed.com · (305) 209-8356 · ADHD Telemed, LLC (mailing address: 777 Brickell Ave, Suite 500, Miami, FL 33131).
